Thornaby Pavilion, Thornaby Pool and Northfield Sportsdrome.
The programme is a service provided by Stockton Borough Council/Tees Active in conjunction with local GP’s, and other health practitioners to help you manage and control your medical condition, health, lifestyle and well-being.
Types of activities included are:
- Light exercise classes
- Water based activities
- Health walks
- Tai Chi
- Chair based/postural stability exercise classes
(Although the scheme is heavily subsidised by Stockton Borough Council, a nominal charge will be required for each session and consultation).
What happens now?
Once you have been referred to the Active Health programme, your health practitioner will complete a referral form for you, and give you a copy. You will then be asked to contact the reception of the leisure facility of your choice and asked to book a consultation appointment. You will be asked by reception for your name, contact telephone number and the name of the person who has referred you to the scheme.
What happens at the consultation appointment?
The information collected during the appointment allows us to prescribe a course of exercise that is entirely suited to your needs. You will be asked further questions about your health and lifestyle and we will discuss with you the type of physical activity that would be beneficial.
You will be issued with a membership card to produce at reception upon arrival, and on payment for your activity. A further appointment will be made for you to start your exercise programme.
The exercise programme
Depending upon your needs you will be encouraged to exercise between 1 – 3 times per week at a level you can cope with. Throughout the 12 week programme your progress will be monitored, and you will be given continued advice and encouragement. Upon completion of the 12 weeks, you will be asked to complete an evaluation questionnaire, which will help us to evaluate the scheme and continue to provide a high standard of service. Hopefully, at this stage physical activity will have become an enjoyable part of your everyday lifestyle, and you will continue to improve your health and well being independently. Upon completion we will issue you with a leisure membership card to contribute to your long-term participation in physical activity. In addition to this you will be signposted to a range of follow-on activities.
