Active Health

Active Health is a 12 week physical activity programme for adults who have one or more health conditions that are a barrier for them becoming more active. Participants on the scheme are referred by medical professionals who assess that physical activity may be helpful. 

The scheme is designed to help participants manage and improve their conditions, along with their overall health and well-being, through increased physical activity levels, and sustainable behaviour changes.

Duration

12 weeks

3 FREE Consultations

Initial consultation to discuss how we can help you to achieve your goals of becoming healthier and improve your health conditions through physical activity.

There will also be a midway and end consultation to monitor your progress.

FREE Gym Induction

Tour of the gym facilities to show you around and familiarise yourself with the equipment. A bespoke programme will be created to help support and improve your health condition.

Activities

Physical activity recommendations from trained GP Referral Staff. Tailored workout programmes will be built for each individual.

Options to access gym / swimming / group-based classes that are appropriate for patients with a wide range of health conditions.

Price

£3.95 per session

Discounted monthly membership options are also available (our team will talk you through the different pricing options).

Qualifying Criteria

Medical conditions such as cardiac, pulmonary, neurological, musculoskeletal, mental health conditions, cancer rehabilitation. Other health conditions may also qualify.

Specialist Advice

Guidance on options for continuing with physical activity after the completion of the programme to continue to improve health and wellbeing.

Recommended Activities

Our team will create a tailored workout programme for you depending on your medical condition. This could include low intensity exercises in the gym, swimming, group-based classes that are appropriate for patients with a wide range of health conditions as well as exercises at home or outside including walking, running and cycling. Chair Based Exercise Class

These sessions are aimed at individuals who are suffering with neurological conditions or limited mobility. Sessions feature a full body workout with use of light weights, resistance bands and exercise balls all delivered in a chair. Sessions are delivered by a friendly, fully qualified Fitness Instructor.

Medimotion Bike Exercise Sessions

Individual MotoMed exercise bike sessions are for people with limited mobility due to a medical condition. Sessions provide fitness, mobility, strength & condition management, on specialist equipment, in a safe and controlled environment. Please note: an induction is required before use of the bikes, which can be done by the Neurological team.

The medimotion bikes are assisted equipment with passive and active functions meaning that you can start slowly and build up your stamina. With regular movement participants can improve their health conditions.
