What is the acceptance criteria to join the programme?
An individual who satisfies all the following eligibility criteria (and none of the exclusion criteria) is eligible to access the Active Health Programme:
- Aged sixteen years or over
- Resident, or registered with a GP practice, in Stockton-on-Tees
- Motivated to increase physical activity levels
- Patients for whom a medical professional has assessed that physical activity may be helpful to the management or improvement of their condition(s)
- Patients who would find it difficult to access general sports and leisure services or increase their activity levels without additional support
What is the exclusion criteria for the programme?
The ethos of the Active Health programme is to be accessible to as many people as possible, and reasonable adjustments will be made to support people to access it. However, an individual who meets any one or more of the following exclusion criteria is not eligible to access the programme and will not be eligible for referral:
- Unstable Angina
- Unstable or acute heart failure
- Post MI or cardiac surgery e.g. CABG / Angioplasty / Valve Surgery / Pacemaker / ICD
- New or uncontrolled arrythmias
- Resting or uncontrolled tachycardia
- Unstable diabetes
- Resting DBP > 100mmHg
- Resting SBP > 180mmHg
- Symptomatic hypotension
- Febrile illness
- Frailty – Severely Frail Completely dependent for personal care, from whatever cause (physical or cognitive)
- Eating disorder
- Pregnant – unless explicitly supported by their clinician
- Bariatric surgery within the last 6 months – unless explicitly supported by their bariatric physician
How can I join the programme?
You will be required to speak to your GP or Health Practitioner. Once they have assessed your health condition and have deemed that physical activity will help to improve it they will refer you onto the Active Health programme.
Patients referred to the Active Health programme will be contacted by Tees Active within 2 weeks of the referral being made.

Where can I attend the Active Health programme?
The programme is delivered across all Tees Active venues – Billingham Forum, Stockton Splash, Thornaby Pavilion and IB Leisure.
Is there a cost to attend?
The initial consultation, gym induction and 2 following consultations are free and are funded by Stockton Borough Council.
Each session (gym, swim or group exercise class) is priced at a subsidised price of £3.95.

What happens first?
Your journey will begin with an initial consultation which will include a full health check, goal setting and information on how to get started.
How often do I need to attend in person?
It is essential that you attend your free consultations appointments and your free gym induction. In addition to this your health advisor will prescribe an exercise programme relevant to your health condition which will state the recommended amounts of visits.
How will it help me increase my activity levels?
We will work with you to find the right activity that will work for you. Options include gym, swimming and studio fitness classes.
How will I be supported throughout the programme?
Your health advisor will support you throughout your journey on the programme and will be available to offer you support and guidance, answer any questions and update your workout programme. You will receive a free initial consultation, a free gym induction as well as a free midway and free end consultation to monitor your progress.
What will happen after the 12 week course ends?
Towards the end of the 12 week programme you will attend a free consultation appointment. Here you will be offered further support and guidance to help you continue to improve your health conditions through physical activity.
